The Vermont Blueprint for Health is a statewide initiative designed to assist residents with chronic conditions in successfully managing their health through improving the quality of primary care. KNG Health and our partner, Doherty Healthcare Consulting, conducted structured interviews with payers and providers throughout the State and recommended a detailed primary-care payment reform model that could be implemented throughout the State. The model included a value-based purchasing component that would reward providers that met certain standards in treatment of patients with diabetes. The recommendations were ultimately adopted for a pilot program. As part of development of the pilot, Dr. Koenig co-facilitated meetings between stakeholders to reach agreement on pilot parameters, and proposed a payment model for the pilots that formed the foundation of the approach used by the state.
